Schedule Class 1, 2, Class 3 or ATC Health Program Exams

Pre-Appointment Class 1, 2, or 3 Aeromedical Exam Steps:

Please read and follow these steps prior to scheduling your exam with Doc B.

  • Complete the FAA Online Application Process at FAA MedXPress, and complete Form 8500-8 online.

  • Bring a printed copy of your completed Form 8500-8 with you to your appointment.

  • Generate a confirmation number to make an appointment below. 

Pre-Appointment HIMS Program Steps:

First, e-mail Doc B. at docb@flydocb.com before making an appointment to ensure I am available to help.

Pre-Appointment ATC Health Program Steps:

Complete the FAA Online Application Process at FAA MedXPress, and complete Form 8500-8 online.

  • Bring a printed copy of your completed Form 8500-8 with you to your appointment.

  • Generate a confirmation number to make an appointment below. 

  • Bring your FAA Authorization And Invoice for Medical Service or ensure it is e-mailed to doc@flydocb.com prior to your appointment. Note: No FAA authorization = no service.

  • Bring FAA Form 3900-7 (i.e. ATCS Health Program Report by Examining Physician) or ensure it is e-mailed to docb@flydocb.com prior to your appointment. Note: This is your proof of having had an examination.

  • If you have an FAA medical status report for medical conditions such as hypertension, diabetes, gout, etc., be sure to bring a copy of your FAA medical status report with you to your appointment. Note: This is required by the FAA and without it your exam may be delayed.:

Special Instructions: For chaperone requests and/or special scheduling needs, please email Doc B. at docb@flydocb.com at least 7 days before your appointment.

NOTE:

  • A MedXPress online application is required by the FAA for all applicants.

  • The MedXPress application is not official until imported and verified by Doc B. Changes to your application can be made at the time of your examination.

  • Routine uncomplicated Annual physicals and Dental exams do not need to be recorded in your FAA MedXPress.
